Milk thistle (Silybum marianum) is one of the most well-researched liver herbs, primarily due to its active complex silymarin, which directly protects and helps repair liver cells. It works by stabilizing hepatocyte cell membranes—essentially making it harder for toxins (including alcohol metabolites like acetaldehyde) to penetrate and damage the liver. At the same time, it acts as a strong antioxidant, reducing oxidative stress and lipid peroxidation, which are major drivers of liver inflammation and fibrosis.
One of its most important actions is that it stimulates protein synthesis in liver cells, helping promote regeneration and repair of damaged tissue. Research—both clinical and pharmacological—has shown benefits in conditions like alcoholic liver disease, fatty liver, and toxin-induced liver injury, with improvements in liver enzymes (ALT, AST) and overall liver function markers in some studies.
